Key Accountabilities:
Accountabilities Percentage of Time (%) 1. To supervise/take the lead role to assess, design, plan, implement and support care plans/management of programs and activities as agreed with the line manager in order to maximise individual service user opportunity, development and outcomes / goals 2. To take ownership to establish, develop and sustain effective working relationships with all stakeholders including service users and their carers and use your initiative to strengthen relationships. 3. Provide appropriate, practical assistance and support to meet the needs and requirements of service users, individually and as a team. 4. To provide support and manage staff performance through supervision sessions in order to further develop standards of practice within the team. 5. As agreed within the service to under take the role, duties and responsibilities of a key/case worker if required 6. Ensure that service users have appropriate access to advocacy and where appropriate to the service, are empowered to self-advocate and be self-determining. 7. Actively support and promote the inclusion and participation of service users using a range of interventions e.g. facilitation, user participation, appropriate development activities. 8. As discussed with the line manager, undertake relevant training and contribute to team development through workshops and other learning opportunities to promote good practice. 9. Work in partnership with other professionals, carers and professional agencies involved in supporting service users and their families. 10. Provide an active and constructive lead within the team and be involved in planning and implementation of care and support that achieve positive outcomes for the service, service users and their carers. 11. Comply with the Council’s financial regulations and to take responsibility for the allocation and use of resources available within the specialist area, including other sources of funding so that maximum benefit is obtained for service users within budget. 12. Work effectively to meet goals and targets and work to required deadlines, adhering to Departmental/service specific procedures, systems and policies i.e. Medication Policy the Council’s Code of Conduct and related policies i.e., Equalities & Diversity, Data Protection, Confidentiality, Information Governance, safeguarding and MCA 13. Comply with the Council’s financial regulations and to take responsibility for the allocation and use of resources available within the specialist area, including other sources of funding so that maximum benefit is obtained for service users within budget. 14. Comply with a range of statutory requirements and legislation as directed by the line manager. 15. Comply with the Council’s data management policies and maintain accurate and up to information using the electronic recording system. 16. Corporate Health and Safety Responsibilities
All employees have personal responsibilities to take reasonable care for the health and safety of themselves and others. This means:

Job Specifics – Skills, Experience Essential:
- Equivalent vocational experience of NVQ Level 4 or above
- Knowledge and experience of delivering and reviewing personalised services that focus on Independence, Wellbeing and are outcome focused to vulnerable adults with a range of care and support needs including behaviour that challenges.
- Ability to support staff through supervision including setting targets and monitoring individual performance against service objectives and competencies.
- Ability to manage own work load, be flexible, work with others within the team to meet deadlines and be pro-active in meeting the needs of the service and stakeholders
- Knowledge of current legislation around Adult Social Care including Safeguarding.
- Excellent IT skills and an ability to produce reports and letters
- Ability to cover out of hours on call on a rota basis
Desirable:
- maintain and develop accurate records within council electronic systems and onsite records including finance
- Experience of developing staff rota remaining within budget envelope
